In low-income countries, most caries cases go untreated. This leads to tooth loss - and, as a result, digestive problems and other negative consequences.

Caries can significantly impair quality of life: for example, its presence can cause discomfort during eating and sleeping, and in the later stages (when abscesses occur) can lead to pain and chronic systemic infection. Caries has a negative impact on economic development - people who experience toothache are more likely to miss work and school.

Growing enamel directly on the tooth

If scientists cannot directly influence the growing consumption of sugar, then creating solutions for more effective and painless dental treatment is in their area of ​​expertise. The last major breakthrough was made by scientists from Zhejiang University - they developed a method that allows you to grow enamel directly on the tooth when treated with a special gel.

For the first time in the history of enamel building research, scientists have used a building material carrier to restore enamel, amorphous calcium phosphate. They were made by a fairly simple and inexpensive substance - triethyl acetate.


The mechanism of growing enamel on the tooth, developed in China

To test the method, the researchers first treated the tooth with acid, severely damaging the enamel, and then tried to restore it by applying a gel with calcium phosphate and a carrier. Observations have shown that due to triethyl acetate, amorphous construction material is included in the composition of old enamel crystals and causes them to grow.

During the experiment, scientists managed to build up a tooth by 0.0027 mm, while for use in real practice, it is necessary to build up at least 0.5 mm of enamel. At the same time, the mechanical properties of artificial enamel did not differ from the real one, and the extension process can be repeated an infinite number of times. This means that the issue of introducing a new method into real practice depends only on how quickly scientists can speed up the process.

Treatment without drilling and fillings

Researchers from King's College London have developed a different approach to restoring teeth - the method allows you to get rid of caries without a drill and fillings of amalgam or composite resin, which are usually filled in the cavity in the tooth after treatment. The method is called Electrically Accelerated and Enhanced Remineralization (EAER) and involves accelerating the natural process of moving calcium and phosphate minerals into the damaged tooth.

Before starting treatment, doctors prepare damaged tooth enamel, and then, using weak electrical impulses, calcium and phosphate are delivered to the tooth. Materials gradually fill the cavity, forming a new enamel. Researchers claim that in this way it will be possible not only to treat caries, but also to whiten teeth.

In 2014, the authors of the development created the Reminova company to commercialize the method and promised that it would be available to the general public within three years. However, until now, only a few clinics in the UK can treat teeth with EAER.

In 2017, their colleagues at King's College proposed another method of dental treatment without fillings. It involves stimulating the natural self-healing ability of teeth through the activation of stem cells in the soft pulp.

The pulp is a loose fibrous connective tissue with a large number of nerve endings, blood and lymphatic vessels that fills the cavity of the tooth. This tissue helps to stimulate the formation of dentin during the carious process. In addition, the pulp is a biological barrier that prevents the penetration of microorganisms from the carious cavity through the root canal outside the tooth into the periodontium.

Under natural conditions, this process allows small cracks and holes in the dentin, the hard part of the tooth located under the enamel, to be regenerated. Scientists used tideglusib, a drug that is used as a treatment for Alzheimer's disease and is safe for clinical use, to force the tooth's own cells to repair cavities that extend from the surface to the root.

This method does not yet allow you to refuse drilling, but does not require the installation of a seal. During the treatment, the doctor removes caries with a drill, and puts a biodegradable sponge with a preparation in the resulting cavity, which stimulates the restoration of the original structure of the dentin.

Tests on mice showed that the tooth with the cavity filled with the drug gradually recovered without additional intervention.

Regeneration of teeth after extraction

It is known that the tooth of a modern person is not able to grow back after removal. However, Neanderthals probably had a mechanism for the complete regeneration of teeth - such a hypothesis was put forward by scientists from the University of Southern California.

By analyzing the molars of a modern human ancestor, the researchers found epigenetic regulators that allowed teeth to regenerate. It's about about the Ezh2 protein, which helps develop the bones of the face.

The analysis showed that the teeth of Neanderthals had a very long root shaft, and the enamel and root were destroyed much more slowly than in modern people. The reason for this is probably the effect of diet or physical activity on certain proteins, in particular on Ezh2.

The researchers tested the mechanism of the protein in laboratory mice. During the experiment, geneticists removed Ezh2 from the molars of rodents and followed their development. The study showed that the balance between Ezh2 and Arid1a allows restoration of the tooth root structure and proper integration of the roots with the jaw bones.

The balance of these proteins affects the structure of the facial bone and teeth, not only in rodents, but also in humans. Now geneticists are studying how much protein is required for the regeneration of a human tooth.

Now it will be easier to treat teeth?
